How do you handle movement in combat? Is it all simultaneous, does it all start at the same time? I know combat is broken down by rounds to make it more manageable, but I don’t like the idea that everything starts fresh at the beginning of the round. I generally have every new action start on the segment of the round indicated by initiative. It seems more like a rolling combat to me. I consider the segments of the round before your action begins as carry over action from the previous round, assessing the situation, decision making, etc. And then your action starts on the initiative segment. This is only for starting new actions. Continuing to do what you had been doing in the previous round, starts at the beginning of the round.

I pretty much have all movement begin fresh with each round, mainly to keep track of where characters are in relation to area effects. The round remains abstract enough so that characters are not literally acting in a given segment, whether first or some other, but the presumption is that if somebody moves for 10 segments in a round his action finishes at the end of the tenth segment.

I've played it both ways. Current;y my initiative rules are pretty flexible based on the situation, but I prefer to not have actions start at the beginning of each round. I will also try to string a series of actions together that may last more than a round if the PC has a plan. In that case one phase of the plan just rolls into the next similar to the extended plan the player gives in the "Action During Combat And Similar Time-Important Situations" om p. 71 of the DMG.

I was rading over some initiative-themed posts by Gary recently, and I came across one where he stated that he allows casters to cast and move in the same round. I think I would be willing to give this a try. Gary stated he would give a number of segemnts worth of moving equal to the balance of the segments -1. I think I'll give this option a try in my next game.

Yeah, I am familiar with those posts, and as a point of interest it actually reflects the last published first edition Sage Advice, which says pretty much the same. Presumably, that was standard practice at Lake Geneva by then. Given that characters can take up to ten segments of action in a given round, it makes sense.
