You're right, as DM that's the only fair way to referee it -- and scouting is an important tactic to learn. Then again, many a scout ends up like that gnome did in the "Example of Play" in the DMG Sample Dungeon.
DM: "The rest of you see your scout pulled into the shadows by a long gray arm. Then you hear a muffled shriek, which cuts off abruptly. Nasty gobbling and rending sounds ensue. Body parts you don't quite recognize get thrown about -- what looks like intestines and perhaps part of a liver -- you're just not sure. In ghoul-language, you hear one shadowy figure gibber to another, 'Please pass the salt.' 'Why, certainly,' the other replies. The gory feast continues. What do you do?"

But one dead party member is still better than a TPK.

No argument here. That's why I was always trying to get Magrod or Sancho to scout ahead. But often no player wants to be the scout.

A few thoughts: I'm no fan of the art, and I find the players' illustrations to be pretty much useless. I'm going to disagree with Scott here. I like Jason Walton's style very much. And for me, this art will be very excellent for my gaming group who are young and less experienced at RPGs than most. ;D

I like Jason's work too. With the players illos, it was more of the presentation than the art itself.

OK, just finished a thorough read of Mouths of Madness. Man it’s a great work. And I really enjoy Gary’s recommendations and comments about how to do/run certain encounters and situations. You veterans probably don’t need much of that, but I find them helpful and they spark my imagination for alternative methods or completely expanding on his recommendation to create an in depth encounter. I can’t wait to run this. I think my players are going to be blown away at how much cool challenge, atmosphere (for me, fun) and interesting things they are going to find/encounter. It was also a load of fun to re-read Caves of Chaos and compare stuff.

Wow, thanks Rhuvein, that's encouraging to hear. I look forward to hearing how you use the adventure and incorporate your own wild and crazy ideas! As regards the future set, I'm afraid I am not allowed to provide any details or information. Sorry. Best, Jeff T.

Hey, I just noticed Mark Ratner got a 'special consultation' credit in this. Good to see ol' Ayelerach providing some input.

He was a player in Gary's campaign. He was with Erac's Cousin when he freed Frazz Urb Luu.

I'm back from a wedding in Cancun, so I should be able to wrap up the review some evening this week. Sorry for the delays, gents, but as others have said: it's a lot of material to cover, and I want to do it justice

In Scott's Campaign: Our little hamlet of Oakhaven* on the Urt River north of Yggsburgh, has been overrun by a small army of evil humans and humanoids. We have fled to Yggsburgh, and now are about to prepare our first expedition to the Upper Works of Castle Zagyg... Someday, perhaps, we will amass enough wealth and power to return to Oakhaven, and get revenge -- destroy the nearby ruined Temple of Hextor once and for all -- and drive out the wicked folk from the north...
*I did not find Oakhaven on the Darlene map of the East Mark, but I am guessing it is near outdoor encounter area 15 or in the northern Little Hillwood region.

Oakhaven is my own creation. It is northwest off the map, up the Urt River.

Once Thornold is healed up enough, he will try to find Elmreth -- trying to recall whom Elmreth said he intended to speak to in Yggsburgh about getting troops to come to the aid of Oakhaven. I assume he would go to the Mayor of Yggsburgh to make such a request.
